Issuing identity cards for the hearing impaired persons above 3 years of age to establish their identity.
Three copies of the 3cm*2.5cm sized (size of the national ID) colored photos are required and two copies of them should be sent in an envelope with the application.
The other photo should be pasted on the allocated place of the application and it should be certified by one of the below mentioned.
- If the applicant is a resident of a division, the photo should be certified by one of Divisional secretary, Social service officer, Development officer or Grama seva officer.
- If the applicant is a student of a school. The photo should be certified by the principal.
- If the applicant is a trainee of a social welfare institution, the photo should be certified by one of social welfare authority or the officer in charge of the institution.
An accepted medical certificate should be needed to confirm the hearing deficiency.
When failed to establish a medical certificate, at least one of below mentioned documents should be introduced.
- A certificate obtained by a school or vocational training institution for hearing impaired.
- An acceptable hand written certificate from a chairman or secretary of an organization for hearing impaired.
- A document establishing the hearing impairment issued by the Grama Niladhari of the division.
No any fee will be charged for issuing the identity card.
